Here’s How We Can Help

The Serious Illness Care Program offers extra care and support focused on you. You will have a nurse who calls you to:

  • Help with pain and other problems
  • Schedule (coordinate) your doctor appointments
  • Talk about what matters to you
  • Teach you about your condition
  • Support you and your caregiver

You are the Expert in You

If you become seriously ill, only you will know what matters most to you. It’s important to share these thoughts with your doctors and loved ones so they can give you the care you deserve.

Here are three ways to share what you care about:

  1. Choose a Trusted Partner: Pick someone who can speak for you if you can’t
  2. Fill Out a Living Will: Write down your wishes in a living will. If things change, you can update it.
  3. Talk to Your Doctor: Tell your doctor what matters most to you and who will speak for you if needed.

Living Life on Your Own Terms

When facing a serious illness, it’s important to have a team by your side. We’re here to help you manage your serious illness with a personalized care approach. People who include palliative care in their treatments often experience:

  • A better quality of life
  • Less physical pain
  • Improved mood

Palliative care helps you talk more openly about your illness, make smart choices about treatment and understand what to expect. It also assists you and your family in managing treatment symptoms and side effects.

Palliative Care: Extra Layer of Support When It Matters

Palliative care is a special medical treatment for people with serious illnesses. Our team of doctors, nurses and specialists works with your care team to give you an extra layer of support, focusing on easing symptoms and stress.

The goal is to improve the quality of life for you and your family by meeting your needs, not just focusing on your illness. You can get this care at any age or stage of a serious illness, along with your regular treatment.

If you often visit the ER, get admitted to the hospital, have side effects from medicine, lose weight or struggle with daily tasks, this program is just right for you!
